Making a Referral
The Hope Institute for Children and Families serves children ages 5-21 with developmental disabilities, including autism. The Admissions Coordinator accepts referrals from families, schools, social workers, PAS agents, mental health workers, case coordinators and clergy for respite, residential and/or educational services.

What to Include in Referral Packets
The Admissions Coordinator, in partnership with a multi-disciplinary team that includes education, residential and health services professionals, reviews referral packets and oversees the referral and admission process.
To make a thorough assessment of a child, the referral packet must include:
- a cover letter indicating the reason for the referral and the presenting problem, including medical history
- results of a current physical exam
- a list of current medications
- the most recent Individualized Education Plan (IEP) and 3-year case study
- a current social history
- a recent psychological evaluation, including diagnosis and full-scale IQ or functioning level
- information regarding behaviors and the Behavioral Intervention Plan (if appropriate).
A Word About Funding
Residents of Illinois receive funding through one or more of the following sources:
- Illinois Department of Human Services
- Illinois Department of Children and Family Services
- individual school districts, partially funded by the Illinois State Board of Education
- private pay arrangements.
Private pay also is accepted for all out-of-state admissions.
